How will we support your child when they are leaving this school or moving on to another class?

School staff recognise that transitions can be a challenge for a child with SEND and therefore steps are taken to ensure that any transition is as smooth as possible.

  • If your child is moving to another school:
    • We will contact the school SENCO and ensure s/he knows about any special arrangements or support that need to be made for your child.
    • We will make sure that all records about your child are passed on as soon as possible.
    • Where possible we will ensure that the child has opportunities to visit the school at different times.
    •
  • When moving classes in school:
    • Information will be passed on to the new class teacher in advance and in most cases, a planning meeting will take place with the new teacher. All IEP’s or Personal Profiles will be shared with the new teacher.
    • Opportunities are available for all children to visit their new class to familiarise themselves with the classroom environment and teachers/teaching assistants.
    • We will ensure that your child has a familiar adult they can approach during the transition period.

  • In Year 6:
    • The SENCo at Lark Rise will discuss the specific needs of your child with the SENCO of their secondary school.
    • All information will be passed to their new school, including copies of IEP’s or Personal Profiles and professionals’ reports.
    • Where possible your child will visit their new school on several occasions and in some cases staff from the new school will visit your child at Lark Rise Academy. This will allow your child to complete transition activities with staff from their new school.
    • When reviewing EHC Plans, staff from your child’s new school will be invited to discuss their transition.
